The Primary and Community Health Branch within the Health Workforce Planning and Accountability Division has the lead role in setting system level strategic policy directions, funding and accountability related to primary and community health (PCH) and chronic disease management (CDM). The branch is responsible for ensuring coordination and consistency in the development, interpretation, implementation, monitoring and evaluation of health policy or programs relevant to or having an impact on PCH and CDM.

As part of the mandate for implementing programs relevant to and having an impact on Primary Health care, currently the branch leads the implementation of the Primary Care Network (PCN) governance structure to improve Alberta’s ability to achieve integrated collaborative team-base primary health care that meets the needs of Albertans. This includes supporting a provincial PCN committee and five zone PCN committees that provide advice to the Minister regarding PCN- related issues as well as other innovative models/initiatives within the PHC system.
