The Care Coordinator is responsible for co-creating a caring, dignified and trauma-informed experience for all guests and clients at CUPS. This position will provide holistic information regarding CUPS services and external resources for clients to make informed decisions regarding the supports of interest to them. Care Coordinators will work in partnership with CUPS clients to develop care plans that are sustainable and relevant to the client, linking the client to appropriate supports both within CUPS services and externally.
Leadership:
The Care Coordinator will ensure clients receive respectful, trauma-informed care by assisting the client where they are at providing information, de-escalation support, and fulfilling crisis support as appropriate and available.
Operate in synergy with CUPS internal departments continuously assessing client needs, gaps in service, care plan modifications and advocating for the voice and self-determination of the client
Facilitate a healthy and safe work environment through learning and enacting all CUPS health & safety protocols, while maintaining an active awareness of clients and colleagues
Promote a culture that reflects CUPS vision, mission, philosophies and values
Organizational Alignment and Performance:Deliver CUPS clients and guests with information and education of services both internal and external of CUPS available to them in a manner appropriate and effective for the client whether in person, phone or over email
Assist clients in defining initial strengths and needs through CUPS standardized assessment and track progress with the client via follow up assessments to celebrate achievements and determine further needs
In collaboration with the client, develop a care plan to meet the needs identified and connect the client to services internally and externally to achieve goals
Remain up to date on internal changes to programs, services and personnel to ensure accurate care plan development and warm hand-offs of clients to colleagues
