Longevity, negative lifestyles, changing expectations from people and communities, mental health needs, vulnerable populations, and advancements in technologies are leading to new challenges in health and social service delivery. These challenges include increased prevalence of multiple chronic conditions, rising demand of services, increased emergency admissions, care that is reactive rather than proactive, and people getting lost between the “cracks” of the system.

Governments, policy makers, and provider networks across the world are looking to transform the way health and social services are delivered so as to better meet the needs of people today and achieve positive health and well-being outcomes. In particular, health and social services systems are looking to better coordinate and integrate their services around the journey of those using services.

To address this need, HSO will be launching the “first of its kind” HSO:76000 Integrated People-Centred Health Systems Standard.
