    Patch is best described as a neighbourhood hub that provides a wide range of services to children, families and individuals in their community. The central goals of the Patch hubs are to decrease poverty, increase health and wellness of children, families and individuals, and build thriving communities. The Patch hub acts as a central access point for services that reflect a holistic response to the effects of poverty and that are provided in a way that is reflective of the local needs of the community.

    The Position:
    Patch is looking for a full time, Community and Family Facilitator (CFF).

    The Community and Family Facilitator is part of a team responsible for the day-to-day operations and walk-in appointments at the Patch hub, and is assigned to deliver one of four specialized services: Family System Navigator, Connections and Gatherings, Employment and Education, or Financial Empowerment. A suitable candidate with interest and relevant experience in any of the specializations will be considered.
