    The Consultant in Equity, Diversity and Inclusion (Health Educator II), is a member of the Chronic Disease Management Team, supporting and working collaboratively within Community Health Services and with community partners, under the direction of the Manager, Chronic Disease Management. The consultant monitors health, equity and diversity trends and fosters key internal and external partners/agencies with the primary aim to improve programs & services towards equitable health outcomes for all. The Consultant works to support and enhance program delivery and planning to specifically improve health equity for vulnerable, marginalized groups in Edmonton Zone communities. Using trauma informed and equity/social justice approaches, the consultant assesses, plans, implements and evaluates client interventions with key stakeholders towards the aim. With credentials and certification related to diversity, inclusion and change management, along with experience in equitable program design and evaluation, and adult education, this consultant will provide support, education, tools and resources to relevant stakeholders and diverse communities working toward equitable health outcomes. This position works with community members, diverse clinical program leaders and managers across the Zone.

    Description:
    As a Health Educator II, you will participate in the development, provision, implementation, enhancement, and evaluation of new and existing programs and services. You will collect data and discuss health concerns and strategies with program consultants, frontline staff, physicians, managers and community primary care providers. In this leadership role you will teach, mentor, and coach staff as well as provide health and wellness education to the public, patients, and providers.
